Shared Savings and Losses

Quality Measure Performance

 Quality Measure Performance will be determined and reported after Performance Year 1 and in the timeframe determined by CMS Innovation Center. More information can be found at Kidney Care Choices KCC Model 

Kidney Disease Education

 According to the Kidney Care Choices Model Operating Guide: Benefit Enhancements, the KDE benefit is intended to ensure beneficiaries are informed about the effects and treatment of kidney disease, diet and nutrition, transplantation, dialysis modalities, and vascular access. The education benefit is crucial to increasing rates of transplants and home dialysis and delaying the onset of ESRD. The education benefit has been expanded to include beneficiaries with CKD 4, 5 and the 1st 6months of ESRD. 

Post Discharge Home Visits

 Evidence suggests that home and domiciliary visits reduce the likelihood of readmission within 30 days of discharge from an inpatient stay for patients with chronic conditions. The research also indicates lower clinical risk for inpatient admission and lower costs for high-risk patients at 6-month follow-up. This Benefit Enhancement encourages providers to expand home visits for beneficiaries with multiple chronic conditions and a recent discharge from a health care facility.

 Continued availability of telehealth benefits for attributed beneficiaries of CKCC.
